Verified Internship Program Monthly Evaluation for Interns

Twice a month, throughout the duration of the internship, both the club’s internship supervisor and the student member intern must complete a face-to-face check-in meeting and complete the monthly evaluation form (one for the supervisor; one for the student). Though we require two meetings a month, you only need to submit this form once a month. These forms will be submitted to CMAA for review. Forms are due to CMAA no later than the 30th/31st of each month.
