How has this idea enhanced your club's operation, etc.?

Recognizing the importance of providing staff with a simple and discreet way to report a safety issue, a workplace concern, or a potential policy violation, Farmington launched a digital Report an Incident form. To ensure ease of access, QR code signage linking directly to the form was created and placed in key staff areas across the Club. This allows team members to quickly access the form from their phones at any time. Once submitted, the form is automatically routed to the appropriate supervisory team based on the nature of the incident, the individuals involved, and the department affected—ensuring timely follow-up and resolution.

How was this idea implemented, and what have been the club members' reactions?

The form was created and made easily accessible to all staff members through internal communication channels. It was designed to be user-friendly, mobile-friendly, and non-intimidating—encouraging greater usage, especially from employees who may feel uncomfortable bringing concerns up in person. The supervisor team receives the submissions directly and responds accordingly based on the information provided. The availability of this form has improved awareness around workplace expectations and accountability.
